Preparing For Your First Visit

  • First Visit Information:

     Your first visit at Clements Family Dental includes:

    A review of your completed dental and medical history forms.

    Annual X-rays of your teeth.

    Oral examinations to make sure your teeth and gums are healthy, as well as a periodontal screening for periodontal disease.

    One of our hygienists will also clean and polish your teeth at your first visit. If gum disease is discovered or if you have mouth pain that needs to be addressed immediately, we may need to schedule your cleaning for the second appointment.

    Once we have reviewed your dental and medical health history forms and the dentist has completed your dental exam, the dentist will provide you with a dental consultation and information about any additional dental treatments you, or your child, may need.

  • What You'll Need:

    Please bring the following with you to your first dental appointment:

    Your Completed Registration and Policy Forms (found below)

    Your Dental Insurance Card and Photo ID

    Any previous x-rays or dental records

    Name and phone number of your primary physician

    Information about any need for premedication prior to your appointment


 Financial Policy

We accept the following forms of payment: Cash, Check, Visa, MasterCard, Discover and CareCredit (described below).


Payment for services is due at the time services are rendered unless prior arrangements have been made with the doctor and the billing receptionist.


The parent that accompanies the minor child/children to the appointment is responsible for any payment due. For unaccompanied minors, non-emergency treatment will be denied unless charges have been pre-authorized before the appointment date or previous arrangements have been made with the doctor and billing receptionist.


Checks that are returned to our office from your financial institution are subject to a $20.00 returned check fee. This fee covers the processing fees that are charged to our office.


Scheduling & Office Hours

Clements Family Dental is open Monday, Tuesday, Thursday, and Friday from 8am-5pm. We will attempt to schedule your appointment at your earliest convenience. If you are currently experiencing pain or an immediate emergency situation, we will make every attempt to see you that day.

Cancellation Policy

We have reserved dedicated time in our office for your dental appointment. If you find it necessary to cancel, please contact us at least 24 hours prior to your appointment time. Without proper notice, you may be charged a $25 fee which is subject to change without notice.
